The Our Dementia Journey Journal (ODJJ) is a practical resource designed to support individuals living with dementia, their caregivers, and care providers. Created through a collaborative process with families, healthcare professionals, and community members, the journal helps strengthen communication, improve care experiences, and capture meaningful moments throughout the dementia journey.

To reflect the diverse needs of communities across Canada, the ODJJ is available in multiple formats—including paper and mobile app—and has been culturally adapted for First Nations and South Asian communities. It is also offered in multiple languages to ensure greater accessibility and inclusion.

Built with the same collaborative spirit as the original journal, the app makes it easy to document updates, track changes, and communicate with others in the circle of care—whether you’re at home, in a care facility, or supporting a loved one from afar.

Community is at the heart of the ODJJ app. It allows for live-sharing across multiple users, helping everyone involved stay informed, aligned, and engaged in the caregiving journey.

The Our Dementia Journey Journal (ODJJ) is available as a free digital download that you can print and use in a way that works best for you and your care team. We’ve worked alongside caregivers, care providers, and community members to develop versions of the journal that reflect different cultural perspectives, caregiving traditions, and languages—including adaptations for First Nations and South Asian communities.

There’s no one “right” way to use the ODJJ—you can adapt it for personal reflection, care planning, communication among family members, or shared journaling within a team. Use it at home, in long-term care, or in your community program.
